Breast implants are commonly used both in breast reconstruction after mastectomy and in cosmetic breast augmentation to enhance natural breasts. Sometimes, however, breast implants do not work out. The most common reasons for implant problems include capsular contracture, infection, and rupture. Occasionally, other problems can occur, such as extrusion (implant erodes through the skin) or implant displacement. Some patients also report autoimmune symptoms that they associate with the silicone shell.

When implants are removed, they should be removed with complete en bloc capsulectomies so that there is no trace of the original implant or the surrounding scar tissue (capsule) after removal. Additional procedures such as mastopexy, fat grafting, implant replacement, or even natural tissue free flaps can be performed to restore the breasts to a more natural shape and size. If a patient has very little or no breast tissue and proportionally large implants, however, it may be difficult or even impossible to restore their breasts to a normal appearance without another implant or a natural tissue free flap. Patient should keep this in mind before making the decision to remove their implants.
